You’ll get a welcome call about Cologuard to learn more about collecting and returning your sample. Cologuard will show up on your caller ID.
2. CONFIRM YOUR ORDER
Now that your healthcare provider has ordered Cologuard for you, we need to confirm your contact information and where to ship your kit. We may also confirm insurance and billing information.
If your healthcare provider has given you a prescription for Cologuard and you have not yet received a call from the Cologuard team at Exact Sciences Laboratories, please call our Customer Care Center at 1-844-870-8870. Our support specialists will make sure the prescription includes everything we need to complete your order.
3. WHAT TO EXPECT
Now that the Cologuard Kit has arrived at your home, it's time to collect your sample (your sample should be no larger than the bottle of liquid in the kit). It will include instructions for collecting a sample, instructions for returning the kit, a prepaid return label, and our phone number, in case you have any additional questions.
4. SHIPPING AND RETURNING
When you’re ready to ship the kit back, you can drop off the resealed box at any UPS location or call UPS to arrange for a pick-up. No payment required! The sample must be received by Exact Sciences Labs within 72 hours after collection. Since weekend mail may be delayed, we recommend you ship it sometime between Monday and Thursday.
5. PAY YOUR BILL
If you’ve received a bill from Exact Sciences Labs, you can mail in your payment to: Exact Sciences Laboratories, LLC, Patient Pay, 27280 Network Place, Chicago, IL 60673-1272.
