An Evening with Harry Connick, Jr. and Jill Connick
Harry and Jill are travelling the country to bring awareness to something important to your health – getting screened for colon cancer. Join us along with some special guests, PGA TOUR legends Jerry Kelly and Tom Lehman, a colon cancer survivor. The evening will be full of good conversation, a little entertainment and a few surprises from the stage! It’s a serious topic – but we’ll have some fun, too. All attendees will receive 50% off a ticket to the PGA TOUR Champions Charles Schwab Cup Championship, held November 8-11 at Phoenix Country Club.

**Health Care Professionals are considered to be individuals or entities involved in the provision of health care services and/or items to patients, who or which are or in a position to purchase, lease, recommend, use, arrange for the purchase or lease of, or prescribe Cologuard in the United States.
***Beneficiaries of federal and state health care programs (e.g. Medicare, Medicaid, V.A.) are not eligible for this contest. Void where prohibited by law.
Cologuard is intended for the qualitative detection of colorectal neoplasia associated DNA markers and for the presence of occult hemoglobin in human stool. A positive result may indicate the presence of colorectal cancer (CRC) or advanced adenoma (AA) and should be followed by diagnostic colonoscopy. Cologuard is indicated to screen adults of either sex, 50 years or older, who are at typical average-risk for CRC. Cologuard is not for everyone; not for high risk individuals, including those with a history of colorectal cancer and advanced adenoma, IBD and certain hereditary syndromes. Positive Cologuard results should be referred to diagnostic colonoscopy. A negative Cologuard test result does not guarantee absence of cancer or advanced adenoma. Following a negative result, patients should continue participating in a screening program at an interval and with a method appropriate for the individual patient. Cologuard performance when used for repeat testing has not been evaluated or established. Rx only.
